Viktor Claesson came off the bench to score a late winner for Sweden deep into added time after Poland had fought back from 2-0 down to draw level.
Sweden took immediate control of their final group-stage fixture as Emil Forsberg angled a precise left-footed finish inside the far post in just the second minute, before finishing off a rapid counter-attack after half time to double the advantage.
Bayern Munich striker Robert Lewandowski somehow failed to score from a corner later in the first half. First, his powerful header into the ground rebounded back out off the crossbar, where he met it for a second bite of the cherry - only to again strike the bar with a header from three yards out with Swedish keeper Robin Olsson helpless on the ground. The second rebound dropped agonisingly between his legs and Grzegorz Krychowiak’s as they desperately tried to turn it over the line to no avail.
Lewandowski made amends just after the hour mark, moments after Forsberg’s second of the match. Piotr Zielinski threaded a sumptuous ball through for Poland’s all-time top goalscorer, and although the defence shepherded him out wide, he bent an unstoppable curler into the far top-right corner.
With time running out, he was again on hand - this time left free in the six yard box to swivel and guide home an equaliser, before Claesson sealed all three points with a one-on-one finish.
Sweden top the group unbeaten on seven points, while Poland are knocked out after collecting a solitary point.
